Abstract
In this chapter, de Bont and Zwart analyze the working of a Transitional Incident Prevention Program (TIPP) designed to improve the safety of patients during their transition between primary and hospital care. The chapter systematically describes what the TIPP entails and how its elements link to the outcomes of the program achieved so far.
