Abstract

More than 1.9 million patients in the Netherlands suffer from cardiometabolic disorders, such as increased risk of cardiovascular diseases, diabetes, hypertension, chronic kidney damage, obesity, and dyslipidemia. Partly due to the aging of the population, this number will only increase in the future. A small proportion of these patients are treated in the hospital-often by people in different disciplines-but for the majority of patients, cardiometabolic care takes place at their GP's practice.
