Resmetirom in the Management of MASLD: What Clinicians Need to Know

Abstract

Purpose of Review: This review underscores the importance of appropriate clinical application and long-term pharmacovigilance for resmetiron. Recent Findings: Resmetirom is a highly selective thyroid hormone receptor-β (THR-β) agonist with pronounced hepatic specificity, which modulate lipid metabolism, thereby reducing hepatic steatosis, inflammation, and fibrosis, while minimizing off-target effects on other tissues. In 2024, the U.S. Food and Drug Administration (FDA) approved resmetirom as the first pharmacologic treatment for non-cirrhotic metabolic-associated steatohepatitis with moderate to advanced liver fibrosis. Summary: Resmetirom is a highly selective THR-β agonist that has demonstrated promising efficacy and acceptable tolerability in phase III trials and received FDA approval as the first drug for MASH treatment. Long-term safety and post-marketing studies with large sample sizes are warranted to monitor additional benefits as well as potential adverse effects.
